Who is Blue Ridge Independence at Home for?

To qualify as a participant in this program, seniors must:

be age 55 or older
be certified by the state of Virginia to meet nursing home level of care
live in our service area
be able to continue living safely at home without jeopardizing health or safety at the time of enrollment

Enrollment will continue as long as desired, regardless of any changes in health status.

Who pays for Blue Ridge Independence at Home?

Our services are funded by Medicare, Medicaid, and private payor sources.

Blue Ridge Independence at Home is a Medicare, Medicaid State option program. If you meet the eligibility requirements and are dually enrolled in Medicare and Medicaid or receive Medicaid only, the program is free of charge. If you receive Medicare only or have private insurance, and meet the eligibility requirements, our team will work closely with you to determine any out of pocket costs.

Your interdisciplinary care team consists of clinicians and other healthcare experts.

The Blue Ridge Independence at Home PACE Center

Our PACE Center, located at 2333 Valor Drive in Winchester, VA, is open to participants Monday through Friday, from 8:30 AM to 4:30 PM.

At the PACE Center, participants conveniently have access to health and personal care services with our interdisciplinary care team on site. Participants are welcome to spend the day with us. We provide nutritious meals and coordinate activities to stay active and socialize.

Transportation to and from the Center is included.
